On February 9 2013, while the band Fisherman’s Friends were preparing for a concert at the G Live theatre venue in Guildford, a four-and-a-half tonne vertically-lifting door dropped onto two members of the group.
Tour manager Paul McMullen, 44, from Stockport, was pronounced dead at the scene.
Singer Trevor Grills, 54, from Cornwall, suffered head injuries and died at St George's Hospital two days later.
An investigation into the incident involving officers from Surrey Police and the Health and Safety Executive (HSE) is still ongoing.
